Within the areas of Wound, Ostomy, Continence Management and Pressure Injury Prevention, the ET Clinical Practice Manager:
· Provides coaching, leadership, and mentorship to nursing staff, peers, students and other members of the healthcare team, regarding evidence-based advanced nursing practice, problem-based needs analysis and strategies for change.
· Conducts learning needs assessments and determines education requirements of the inter-professional team and the organization.
· Develops program specific patient/nursing care standards in keeping with the corporate strategic direction, the College of Nurses of Ontario, the National Pressure Ulcer Advisory Panel, the Registered Nurses of Ontario Best Practice Guidelines, the Canadian Association of Enterostomal Therapists and Accreditation standards.
· Develops, implements and facilitates learning activities that assist staff to enhance their practice.
· Leads the development and revision of evidence-based clinical standards and core competencies.
· Provides education for new staff members at orientation.
· Provides staff development expertise as educators, consultants, facilitators, change agents, and leaders in advancement of nursing knowledge and professional practice.
· Provides resourceful information, through informal and formal conferencing with nursing and other inter-professional care team members in determining appropriate strategies to achieve patient-focused outcomes.
· Determines a treatment plan that incorporates the patient’s goals, needs, support systems, treatment and interventions and required resources.
· Reviews and develops policies and procedures, practice guidelines, and order sets incorporating best practice and research.
· Facilitate ordering and obtaining ostomy, continence, skin and wound care supplies.
· Documents using the nursing process of Assessment, Nursing Diagnoses, Goal Setting, Care Plan Development, Implementation and Evaluation.
· Coordinates the annual Pressure Ulcer Prevalence Study, reviews results and identifies process improvement opportunities.
· Using the Safety Reporting System, oversees the developmental of monthly Hospital Acquired Pressure Injury Statistics in conjunction with the Decision Support Department, and determines priorities and goal setting based on outcomes.
· Develop and maintain links with community partners to collaborate and standardize practice across the LHIN.
· Current registration with the College of Nurses of Ontario
· Bachelor’s degree in Nursing required
· Masters of Nursing Degree Preferred
· Graduate of an accredited Wound, Ostomy and Continence Enterostomal Therapy Nursing Education Program preferred
· Five years recent relevant clinical experience required
· Three years recent experience in developing and delivering educational programs
