Position Summary:
The Responsive Behaviour Navigator is a core member of the partnership between Hotel-Dieu Grace Healthcare and Behaviour Supports Ontario (BSO) throughout Windsor-Essex County. Reporting to the Manager of Specialized Geriatrics at HDGH, this role will be to provide an extensive behavioural assessment for inpatient individuals age 55+ that present with behavioural expressions as a result of a dementia or other neurocognitive deficit(s). The successful candidate will provide non-pharmalogical recommendations to the hospital health care team via capacity building, in-person modeling, observation and follow-ups of clients. Collaborates with hospital inter-professionals and interfaces with placement, community agencies, the BSO System Navigator, GMHOT, BSO LTC Lead Team, client/care partners and BSO LTC Internal Champions to plan for and support the implementation of the behavioural intervention care plans as required. The successful candidate will also provide education and support to all care teams involved in the implementation and application of the plans that are developed both onsite and offsite/in-home as needed. Will provide transitional support and continued follow-up until the patient is stabilized in their discharge destination. BSO Community/Long-term Care Lead Team is essential in this role to ensure positive and effective transitions. Advocates for discharge, when appropriate, and facilitates wrap-around supports to reduce re-admission to ED and/or hospital. Coordinates additional resources, such as caregiver support through BSO System Navigator
Qualifications:
3 years experience working with seniors – Required
Ability to work independently and within an interdisciplinary team
Experience conducting Cognitive assessments/screening tools related to the geriatric population (ie. CAM, MOCA, GDS, Cornell, GAS, etc.)
Good knowledge of mental health/geriatric support services in Windsor and Essex County
Excellent interpersonal and communication skills with all clients, staff, visitors of HDGH as well as our partnership organizations
Clinical skills to assess signs and symptoms of psychiatric illness
Experience with psychosocial and risk assessments
Good problem solving ability, and work well under pressure
Ability to work independently and in partnership with community partners
Proven experience in responsive/expressive behaviours
Proficiency in French language is considered an asset
Valid Ontario Driver’s License, means of transportation and ability to travel on a daily basis for outreach services - Required
There will be an interview conducted with both HDGH and the requisite community partner program (Behaviour Supports Ontario-BSO).
Education and/or Certification:
Bachelor or Master of Social Work, Psychology, or Social Sciences – Required
Current Certification of Registration issued through the Ontario College of Social Workers & Social Service Workers - Preferred
Previous Specialty Geriatric Education (I.e. PIECES/GPA, etc.)- Preferred
