Position Summary:
The Assertive Community Treatment (ACT) Team is a client-centered, recovery-oriented mental health service, delivery model that provides support for facilitating community living, psychosocial rehabilitation, and recovery for persons who have the most serious mental illnesses, have severe symptoms and impairments, and have not benefited from traditional outpatient programs. Using the principles of client-centered care within the context of an interdisciplinary team approach, the Registered Practical Nurse will provide service to clients with severe and persistent mental illness.
Qualifications:
Minimum 2 years of experience in the field of psychiatric/mental health nursing –required
Community Mental Health experience - preferred
Ability to function independently in the community
Valid Ontario Driver’s License, means of transportation and ability to travel on a daily basis for outreach services – required
Must adhere to and provide satisfactory auto insurance documents to the Hospital in accordance with the Vehicle Use for the Provision of Care or Transportation of HDGH Client/Patients Policy – required
Thorough knowledge of nursing principles, procedures and practice in accordance with the Standards of Nursing Practice, for Registered Practical Nurses, of the College of Nurses of Ontario and regulations of the Regulated Health Professions Act.
Good problem solving skills and ability to think independently.
Good knowledge of community agencies
Excellent communication skills; Proficiency in French language an asset
Education and/or Certification:
Graduate of an approved Registered Practical Nurse Program
Current certificate of Registration issued through the College of Nurses of Ontario - required
Certification in Mental Health Nursing - preferred
Current CPR certificate required
