Position Summary:
As part of the inter-disciplinary team, the Pharmacist is responsible for providing pharmaceutical care that entails the responsible provision of drug therapy for the purpose of achieving definite outcomes that improve a patient’s quality of life. The Pharmacist also oversees the drug delivery system that integrates the application of the pharmacist’s specialized knowledge with the distribution of medication to assure safe and optimal medication therapy for the patient.
Qualifications:
Ability to assume responsibility for practices with level of competence, and practice in accordance with relevant legislation, CSHP standards and other relevant practice documents, professional code of ethics and hospital policy - Required
Additional training or credentialing in pharmacotherapy is considered an asset
Ability to work within a fully automated dispensing device distribution system (PYXIS) and central intravenous admixture program
Excellent interpersonal and communication skills with all patients, families and visitors
Must be able to work effectively and efficiently in a fast-paced environment
Demonstrated commitment to professional development
French Language proficiency is an asset
Must be able to work all shifts as required
Education and/or Certification:
Bachelor of Science degree in Pharmacy from a recognized University or College of Pharmacy- Required
Must be licensed with the Ontario College of Pharmacists- Required
Previous hospital experience - Preferred
Actively in Part A of the OCP registry
