Position Summary:

Reporting to the Operations Manager, the Registered Practical Nurse (RPN) is responsible for delivering safe, effective, and ethical nursing care within the HART Hub. Working collaboratively with the interdisciplinary team, the RPN conducts ongoing assessments and monitoring of patient health status, identifies patient and family needs, and contributes to the development and implementation of individualized care plans. The RPN supports seamless patient transitions by coordinating with the Withdrawal Management Department throughout the admission process to ensure appropriate placement within the HART Hub.  



Qualifications:

Minimum one year of nursing experience in mental health and/or addictions - Required

Minimum one year of experience in specialized outpatient mental health service – Required

Demonstrated knowledge and understanding of mental health care, relevant legislation, and the Mental Health Act

Thorough knowledge of nursing principles, procedures, and best practices in alignment with the College of Nurses of Ontario Standards of Practice and the Regulated Health Professions Act (RHPA)

Strong problem-solving abilities with the capacity to work both independently and collaboratively within an interdisciplinary team

Excellent communication skills with the ability to engage effectively with patients, families and colleagues

Proficiency in French language is considered an asset




Education and/or Certification:

Graduate of an approved Registered Practical Nurse Program

Current certificate of Registration issued through the College of Nurses of Ontario – required

Certification in Mental Health Nursing – preferred

Current CPR certificate - required
